Important: Filling out this form does not automatically qualify you for this program. If accepted, each child will receive three items for Christmas. If items are added without the foundation's permission, you will be disqualified from the program. Any verbal abuse, profanity, or threats towards our board members or members of our staff are strictly prohibited and will result in your termination from the program.
